Statement by the legal representatives of the entity

The declaration included in this section will be made by those persons who hold legal representation of the entity, in accordance with the provisions of the specific applicable legislation.

The following information must be completed in this section:

• Identification data of each representative (name, surname and NIF ).

• In the box "Power of attorney date" the date of the power of attorney or the power of attorney deed will be entered.

• In the box "Notary/Others" the notary's office where the power of attorney was issued will be indicated. In the case of civil law companies that have recorded the power of attorney in a private document, the date of that document must be entered in this box.
