Documentation to submit with the application for the authorisation of the professional diesel cards

  1. Powers of attorney that prove the representation of the application's signatory.
  2. Copy or photocopy of the card for which authorisation is requested, in which the following must feature:
    1. The commercial name of the card.
    2. The holder to whom it was issued or company name.
    3. The registration number of the vehicle that can refuel using the card.
    4. The expression “Professional diesel fuel”.
  3. The NIF of the physical or legal entity that will be used to present the list of Diesel supplies.
  4. The name and phone number of the contact persons who will assist with any potential incidents that arise whilst processing said information.
  5. Express statement that the authorisation is requested for the purpose of the use of the professional diesel card as specific mean of payment for the acquisition of the diesel for which the refund is requested.
