Model 030 documentation

Form 030 must be accompanied by additional documentation by checking the following boxes:

Box 101 and 102. High:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) accreditation card.

Box 103 and 104. Address:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.

Box 105 and 106. Address for notifications:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.

Box 109 and 110. NIF application:

  • In the case of a NIF application for a natural person of foreign nationality (NIF M):
    • Valid passport of the applicant or other valid Identity Document equivalent to the National Identity Document of their country of origin that proves their identity, age and nationality.
    • Provide proof that, although it has not yet been assigned, you have applied for the NIE from the General Directorate of the Police or, if applicable, documentation that justifies that you are not required to have a Foreigner Identity Number (NIE)
    • Provide documentary evidence of the transaction of a tax nature or significance that you are going to carry out (for example, employment contract, property purchase and sale documents, university admission letter, etc.)
  • If the NIF application is for a Spanish national under 14 years of age residing in Spain (NIF K), the following must be provided:
    • NIF of the father or mother or legal guardian, legal representative.
    • Family Book (page showing the holder or holders of the book and page showing the child for whom the NIF is requested) or Birth Certificate of the minor.
  • In the case that the NIF application is for a natural person of Spanish nationality residing abroad (NIF L).
    • Valid passport.
    • Document proving your residence abroad.

Box 107 and 108. Modification of identifying data:

  • If you report a change in your personal or identification data, you must provide:
    • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.
    • Documentation proving the requested change in personal data.
  • If you change your tax residence abroad, you must provide:
    • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport if they have NIF M or L
    • Tax residency certificate issued by the Tax Authority corresponding to the applicant's address abroad.

Box 111 and 112. Change of Civil Status:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.

Box 115 and 116. Application for a new NIF card:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.

Box 117. High Notices:

  • National Identity Document (DNI) or NIE (Foreigner Identity Number) card of the applicant, or passport in case of having NIF M or L.

Box 701 Representatives (Section 8):

  • Title of representation. As an example:
    • Minors:
      • To prove parental authority: present the family book or court order, in case of separation or divorce.
      • To prove guardianship, curatorship or legal defense: submit a court order or a certificate from the Civil Registry.
    • Adults with disabilities over whom representative guardianship has been established: to present a court order.
  • National Identity Document (DNI), NIE accreditation card or valid Passport in the case of persons with NIF M and L, of the representative.

Failure to submit the requested documentation or submitting it incompletely or inaccurately may result in file of your request (Articles 88 and 89 of the Regulations for the application of taxes, approved by Royal Decree 1065/2007, of July 27).
