Paper submission of model 121

Please read carefully the notices indicated at the top of the form.

Disabled descendants or ascendants who are related must have a tax identification number. A communication must be submitted for each dependent disabled person or for each large family that entitles them to the deduction.

Fill in the relevant information and click "Validate". Once validated, you can "Generate PDF " to print and present it to the Administration. Please carefully review the notice box for correct presentation.

You will get the document in PDF which will be initially saved in the download directory set in your default browser.
